Nails it!  The only thing I would add to the list is the bizarre phenomenon of people feeling okay with making vaguely (sometimes overtly) racist remarks about my name.  The best was when a nurse told me she 'expected someone with a turban and a dot on their head, not the girl next door! Har har har!'
